Tweety Bird: $10 Student Basic Annual membership just to keep Birdland going

Donald Duck: $20 Basic Annual membership

Foghorn Leghorn: $50 Priority Seating on side leather couches plus Birdland T-shirt(ran out of T-shirts)

Toucan Sam: $100 Membership, VIP front center stage, plus Birdland T-shirt and Birdland umbrella(only umbrellas left)

Big Bird: $200 Membership, VIP front center, 2 Birdland T-shirts, Birdland umbrella, and you pay for the BBQ that week because that’s who you are: Bird Bird, and that’s just how you roll.

 

Toruk: $1000 Membership. That’s Avatar level. Birdland will throw your blue Pocahontas birthday party or whateva’ whateva’ pow wow you want. We will get the band and provide the food. You just bring your blue Na’vi possee and be revered by your friends like Chief Seattle. You can party with your flock of birds like this:(my beautiful friends and birds in Manila not included,lol)

BIRDLAND JAZZISTA SOCIAL CLUB CONTRACT AGREEMENT

PARTICIPANT’S: RELEASE OF LIABILITY & INDEMNITY AGREEMENT

 

I, _______________________________, acknowledge that I have voluntarily applied to participate in the social activities of Birdland Jazzista Social Club.

I understand and agree that my activities may include, but are not limited to: eating, listening and playing music, fitness or exercise classes, visual arts classes, performing arts classes, and special interest classes. I understand and acknowledge that while I am participating in the Birdland Jazzista Social Club acitivies, I will be conduct myself in a lawful and honorable manner.(no fighting, no underage drinking, no loitering and littering on the street or neighbors’ yards and sidewalk, being respectful) For fitness or exercise classes, I agree to provide and wear my own protective body clothing, equipment, or both. I represent that I am: (1) in good physical condition and emotional health, (2) not suffering from any condition, disease, or disability that can hinder or endanger my participation in the social activies.

 

I AM AWARE THAT THE ACTIVITIES MAY INCLUDE CONTACT SPORTS AND VIGOROUS WORKOUTS. I REALIZE THAT FITNESS OR EXERCISE CLASSES, VISUAL ARTS CLASSES, PERFORMING ARTS CLASSES, AND SPECIAL INTEREST CLASSES CAN BE PHYSICALLY DEMANDING, DANGEROUS, AND HAVE A RISK OF SERIOUS INJURY OR DEATH. I AM FULLY AWARE THAT I AM VOLUNTARILY PARTICIPATING IN THESE ACTIVITIES WITH MY KNOWLEDGE OF THE INHERENT RISKS AND HAZARDS INVOLVED. I HEREBY AGREE TO ACCEPT ANY AND ALL RISKS OF INJURY, DEATH, OR PROPERTY DAMAGE.

 

AS LAWFUL CONSIDERATION for Birdland Jazzista Social Club’s permitting me to participate in the social activities, I HEREBY AGREE that I, guests I bring to any Birdland Jazzista Social Club, my heirs, distributees, guardians, legal representatives, and assigns WILL NOT MAKE A CLAIM AGAINST, SUE, OR PROSECUTE Birdland Jazzista Social Club activities, its officers, agents, or staff for injury, death, or damage arising out of the negligence, intentional, or other acts, howsoever caused, by Birdland Jazzista Social Club or by any officer, agent, or staff of Birdland Jazzista Social Club, as a result of my and guests’ participation in the social activities.

In addition, I HEREBY RELEASE, DISCHARGE, AND AGREE TO “INDEMNIFY” (TO COMPENSATE AND TO DEFEND) Birdland Jazzista Social Club, its officers, agents, and staff from and against ALL ACTIONS, CLAIMS, OR DEMANDS that I, guests I bring to Birdland Jazzista Social Club activities, my heirs, distributees, guardians, legal representatives, or assigns now have, or may later have from today, for injury, death, or damage arising out of my participation in Birdland Jazzista Social Club.

I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT TO INDEMNIFY BETWEEN MYSELF AND BIRDLAND JAZZISTA SOCIAL CLUB, AND I SIGN IT OF MY OWN FREE WILL.
